Our business hours are 9:00 am - 2:00 pm, Monday - Friday. Applications submitted outside of these hours will be reviewed the next business day. We review applications in the order they are received, and a reply can take 1-2 business days.
If you wish to receive a spay/neuter voucher, you must complete an online application.
We ask for personal information when you apply but do not it share it and all information is kept strictly confidential. Your form data is encrypted and fully protected throughout the send process.
We reserve the right to make exceptions based on individual need.

Voucher quantities are limited and are aimed at helping Floyd County residents who are struggling to afford the cost of altering their animals.
Vouchers are available for individuals with an income below $32,340 or a joint household with an income below $36,960 and proof of income IS required.
What paperwork is required?
- A copy of your latest income tax return
If you cannot provide a copy of your latest income tax return then you must provide ONE of the following:
- A copy of your last 3 check stubs
- W-2 or 1099 from employer
- SSI/SSDI/SSA benefits letter
- Unemployment benefits letter
- Food stamps or Medicaid eligibility letter

A valid, Floyd County ID is also required to receive a voucher from FCARL, in addition to ONE of the items listed above. The name and address on your application must match the name and address on your ID.
